OSHA Rolling Tower Scaffold and Safety Checklist
- Is the scaffold designed for the intended working load and posted?
- Is the scaffold height equal to or less than three (3) times the minimum base width?
- Are scaffold components and planking in safe condition for use and is plank rated for scaffolds intended use?
- Have competent persons been in charge of erection?
- Have screw jacks been used to level and plumb scaffold (maximum of 305mm (12″ inches) high) on rolling scaffolds?
- Are casters in firm contact with floor (ground working surface)?
- Are casters of proper size (203mm (8” inches) minimum)? Locking device?
- Are casters and all frames locked together?
- Is tower fully braced on both sides?
- Has horizontal diagonal bracing been positioned properly at the base and intermediate levels of 6.1m (20’ feet)?
- Are guardrails, mid-rails and toe boards in place on all open sides and ends?
- Are guardrails, mid-rails and toe boards installed @ 1.83m (6 feet) or above?
- **Are guard-rails installed @ 1.2m (4 feet ) to 2.4m (8 feet) levels?
- Is platform fully planked and toe boards provided where necessary?
- Have brackets and accessories been properly installed:
a. Brackets?
b. Tube and clamp?
c. All nuts and bolts tightened? - Are working level platforms fully planked between guardrails?
- Have personnel been instructed in the safe use of this equipment?
- Are all Pole material and metal components free of corrosion or thinning?
- Have stair units or ladders and guard-rail gates been provided?
- Does wood planking have cleats installed at end supports to prevent sliding?
- Are planks secured to prevent displacement or uplift?
- Are toe boards installed securely for this type scaffold?
- Are wood products free of decay, splits and other defects?
- Have hazardous conditions been provided for:
a. Power lines & grounding requirements ?
b. Un-level working surfaces?
c. Possible wash-out of footings?
d. Are casters properly installed? - If a tarpaulin/sheeting is secured to the scaffolding, has the scaffolding been designed to withstand the additional wind load?
- Have ladders and access gates been installed and tightened properly?
- Scaffold inspection tag attached to scaffold?
a. Signed?
b. Dated?
c. Restrictions noted?
** Note: This means that a 4 ft (1.2 m) high scaffold would require guarding if it is positioned next to an open floor, or elevator shaft, that would allow a fall from the scaffold greater than 6 ft (1.8 m). Guardrails are also required when a scaffold is located next to a sloped surface where a potential fall could be greater than 10 ft (3 m).
Periodic reinspections verify that the scaffold has not been changed and this inspection is valid. Periodic Inspections are documented on the Scaffold Inspection Tag.
